Application for Disbursements from CAD Restricted Account

The Utah Communications Authority has limited funds in a Computer Aided Dispatch Restricted Account which can be used to reimburse PSAPs for a portion of the costs actually incurred by a PSAP in implementing an eligible Shared CAD System. Below is the form for doing so. Please review R174-1-403 of the Utah Administrative Rules, available here, before proceeding.

What is the name of the agency applying for reimbursement?

What is the geographic area(s) served by the Shared CAD System?

When did/will the eligible Shared CAD System become operational, such as a substantial completion, commissioning, or cutover date?

Please upload a proposal, scope of work, or itemized invoice sufficient to show all hardware, equipment, services, or other costs incurred in the purchase of the eligible Shared CAD System.

Please upload bills of sale, receipts, cancelled checks, wire transfer records, and/or other documents sufficient to demonstrate the amounts actually paid by the PSAP for the eligible Shared CAD System.

Please describe the anticipated effect of the Shared CAD System on the 911 call transfer rate for the relevant PSAPs, including whether the anticipated 911 call transfer rate will meet any applicable benchmarks.

Please provide a narrative setting forth the basis of the anticipated effect on 911 call transfer rates described in the preceding question.
